By clicking the "Start Request" button below, you will begin the evaluation request form, which is immediately followed by our payment page. You must complete a separate enrollment form for each student you wish to request an evaluation for. We are an approved Step Up for Students direct provider through EMA, and you will have the option to choose this as your payment method. If you will be private paying, you will be forwarded to PayPal to submit your payment at the time of form submission.  Our current evaluation rates are:

Option A:  Remote Portfolio Review

Standard Delivery: $35 per student; Rush Delivery: $55 per student

Option B: At-Home Testing Service

Standard Delivery: $60 per student; Rush Delivery: $80 per student

We offer year-round evaluations! You are welcome to request a homeschool evaluation with Apple Blossom any day of the year. 

For Option A, with standard delivery we typically are able to return your completed evaluation form within 3 business days of the phone discussion.  We also offer rush delivery for an additional fee, which guarantees delivery of the completed evaluation form within 24 hours of the phone discussion. 

For Option B, with standard delivery we typically deliver your achievement test license within 5 business days of the evaluation request and return your completed evaluation form within 5 business days of the test submission. With rush delivery, we send your achievement test license within 2 business days of the evaluation request and return your completed evaluation form within 3 business days of the test submission.

Our online enrollment form is easy, secure, and offers HIPAA-level security. Please make sure you have the following information available before proceeding to the request form. These will all be required to submit the form:

• Demographic information for you and your child

-  Full name, date of birth, mailing address, email address, phone number

• Student information

-  County of homeschool registration, school year start date and end date, student's primary grade level

• Valid payment method (EMA direct pay -or- PayPal, Visa, Mastercard, Discover, or American Express)

Additional Submission Requirements for Option A (Digital Portfolio Review):

• Work samples from beginning and end of academic year

-  at minimum: 1 full page of work completed in English/Reading and 1 page of work completed in Math from the beginning of the year and 1 full page of work completed in each subject at the end of the year

• Additional portfolio files 

- at minimum: a summary of educational activities completed throughout the past year, including reading titles. This summary may be in any easily comprehensible format, including paragraph, list, or spreadsheet. We offer fillable PDFs that you may use as a template, which can be found on our Templates page.
